Job Title:  CNA/CMA at the Keith Keadle Center for Continued Care
Position Description: Performs non-professional direct care duties following the care plan under the direction and supervision of the Director of Health Services. Assists with providing skills training to maintain abilities of Villagers including, but not limited to; maintaining a safe and clean room, laundry skills, personal hygiene, and interpersonal relationships with their housemates. Maintains a positive physical, social and psychological environment for the Villagers. Demonstrated organizational and communication skills are essential. The ability to multitask and cope with stress in difficult situations is necessary. Participants in necessary skills training for Villagers to meet their goals through the AFLS assessment process.

Job TitleLife Enrichment Coordinator
Position Description:  This position will encompass the case management and activities coordination of the ALC. Plans and implements activities for individual and small/large groups. Assists with QAPI, Care Plan meetings, maintaining records and training of staff.

Position Description: Responsible for managing the villager-directed health plan and supervising the day-to-day villager clinical needs during assigned shift. Assist in providing the day-to-day nursing activities of the village in accordance with current federal, state, and local standards governing the facility, and as may be directed by the Director of Health Services, to ensure that the highest degree of quality care is maintained at all times.
Position Type: PRN (as needed)
Shift: 7:00am –  7:30pm

Job Title:  Licensed Practical Nurse (Weekend)
Position Description: Responsible for managing the villager-directed health plan and supervising the day-to-day villager clinical needs during assigned shift. Assist in providing the day-to-day nursing activities of the village in accordance with current federal, state, and local standards governing the facility, and as may be directed by the Director of Health Services, to ensure that the highest degree of quality care is maintained at all times.

Position Description: The Director of Nursing at DSH will plan, organize, develop and direct the overall operation of the Nursing Services Department in accordance with current federal, state and local standards governing the facility, and as may be directed by the Administrator to ensure that the highest degree of quality care is maintained. Works with department managers of other areas to assure the appropriate and smooth integration of shared functions of resident services.
