Mission of the Social Service Department
The mission of the Social Services Department is to advocate for the villagers and to foster a trusting environment. We strive to promote self-sufficiency through empowerment of person centered services to enhance the dignity and sense of self-worth of all we serve.
What Annandale’s Social Services Department Can Do For You:
- We provide assistance in completing Medicaid/Medicare and Social Security forms.
- Assist in creating realistic goals focused on person centered interest and skill level.
- Goal tracking and measurement of goals along with counseling sessions when goals are not met to standards.
- Conduct individual counseling sessions for Villagers as needed.
- Coordinate behavioral plans working with on-site Behavioral Specialist.
- Advocate for villagers on grounds and off grounds for quality wrap-around services (i.e. support coordination, insurance inquires, employment placement, ID renewal, etc.)
- Assist each villager in understanding financial budgeting and process money requests while balancing needs of villagers and funds available.
- Assist with travel planning and processing of comings and goings.
- Organize case files for adequate case management.
- Facilitate Individualized Service Plan meetings bringing together all departments including but not limited to residential, supported employment, workshop, recreation, arts/academic and activities.
- Report progress through informative monthly case notes complete with goal tracking, general communication, behavioral planning, and activities.
- Keep an open door policy for both villager and support system. This ensures excellent customer service and above than average case management.
